SMCS contracts with Valley Health Connections (VHC), a nonprofit organization and long-time partner specializing in connecting people with needed services. Staff members of VHC will be available at many SMCS sites for confidential, one-on-one consultations.
Screening and Enrollment Assistance
Screening and enrollment assistance for several programs is available. (e.g. Green Mountain Care programs, SMCS charity care programs, VT breast and cervical cancer screening programs, and other social service programs.)
Patient Assistance Pharmacy Programs
We offer assistance in enrollment in and management of patient assistance pharmacy programs for free medications.
Eligibility Assistance Is Available
Eligibility assistance coordinators are available to help you in assessing, planning, and coordinating the options available to you—so you get the health care and other community services you need—when you need them. Please speak with your health care provider or call Valley Health Connections.
