LUDLOW HEALTH CENTER

As a National Committee for Quality Assurance (NCQA) Medical Home, we are here to serve you as your personal care team. We strive to exceed the standards around access and continuity, population and high-risk management, self-management and community care resources. All the while, we work to coordinate your care beyond our own system. Patient experience is extremely important to us, and we measure your satisfaction. You may receive a survey in the mail, please fill it out to help us continuously improve. 

Our on-site services include: primary and preventive care, pediatrics, outpatient mental health counseling, minor skin surgeries, and planned diabetic visits. Lab and digital x-ray services are offered on-site and aid us in providing you prompt diagnosis and treatment. Walk-in care is offered seven days a week, with no appointment necessary; and free blood pressure checks are always available as needed. Local transportation to our office is available if needed.

If you have an unexpected illness and wish to schedule an appointment, please call as early in the day as possible so we can make every attempt to accommodate your needs. If you wish to utilize our walk-in service, no appointmet is necessary. We will make every effort to meet your  needs in a timely manner.

Other medical specialists, including dental care, can be coordinated to provide you high-quality, comprehensive care that meets your personal needs, as well as the needs of your extended care team.

Care will be provided to all, regardless of ability to pay. Financial assistance is available to those who are income-eligible and fees are charged in accordance with a sliding fee discount policy based on family size and income.
